What you'll do

• Provide admissions coverage at facilities in Bourne, Yarmouth, and New Bedford
• coordinate admissions from initial referral through admission
• collect and review clinical and financial information, confirm payer sources, and obtain approvals
• schedule and conduct tours
• complete admission paperwork and requirements
• coordinate with nursing, rehabilitation, social services, dietary, and business office teams
• maintain records and update leadership
• support census goals and referral relationships.
