Outreach Leader — Primary Care

What you'll do

• Build and manage relationships that generate new patients
• own patient acquisition goals and execute territory sales plans
• prospect through networking, outreach, community engagement, and referrals
• guide eligible prospective patients through enrollment
• maintain a lead pipeline and CRM records
• develop community and referral partnerships
• represent the organization at events and health fairs
• follow up with new patients
• collect community feedback
• and collaborate with Marketing, Clinic Operations, and Care Teams.
