What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine eligibility and appropriate levels of care
• develop, implement, and document plans of care
• educate patients and families on self-care, nutrition, and dietary needs
• determine service scope and frequency
• provide preventative and rehabilitative nursing procedures
• apply infection-control precautions
• direct Home Health Aides and LPN/LVNs
• document progress
• communicate changes and outcomes to physicians and care team members
• monitor cases for payer compliance.
