Registered Nurse Home Care Case Manager

What you'll do

• Conduct comprehensive patient assessments and develop individualized home health care plans

• educate patients and families about diagnoses, medications, and self-care
• collaborate with physicians, therapists, and other professionals
• maintain clinical documentation
