What you'll do

• Arrange continuation of care and services for patients
• work with insurance groups to coordinate appropriate patient care
• assess, plan, provide, and evaluate discharge services
• facilitate access to community resources
• monitor, assess, and record patient progress against a care plan
• collaborate with internal and external medical staff and partner organizations
• maintain patient confidentiality.
