Home Health Registered Nurse — Per Diem, West LA

Job overview

The Home Health Registered Nurse role is onsite in the West Los Angeles area, with per diem/PRN flexible scheduling and visits in the community. Pay is $110–$140 per visit. Remedy Home Health Care provides care directly to patients in their homes to support recovery, independence, and quality of life. The nurse delivers skilled home health care, develops and adjusts individualized care plans, and coordinates with physicians, therapists, and the Director of Nursing. Patient and family education and compliant visit documentation support continuity and effective care.

What you'll do

• Provide skilled nursing care in patient homes, including assessments, wound care, medication management, and treatments
• develop and implement individualized home health care plans
• monitor patient progress and adjust plans as needed
• educate patients and families on disease management, rehabilitation, and senior care
• coordinate with physicians, therapists, and the Director of Nursing
• document visits in compliance with home health regulations
• travel throughout the assigned service area.

Why this role
