What you'll do

• Complete social histories and psychosocial assessments
• contribute relevant information to the minimum data set and care plans
• maintain timely documentation
• support residents’ self-determination and explain alternatives when services are refused
• provide or arrange therapeutic support and education for residents and families
• coordinate discharge planning and referrals
• work with the interdisciplinary team to promote resident rights and psychological well-being
• assist with social service program planning and survey preparation.
