What you'll do

• Conduct psychosocial assessments
• identify social, emotional, financial, and environmental needs
• coordinate discharge and transition plans
• address barriers such as transportation, housing, finances, caregiver support, and follow-up services
• provide emotional support, counseling, advocacy, and crisis intervention as appropriate
• connect patients and families with community resources
• collaborate with the care team
• document in the electronic medical record
• monitor needs and manage multiple cases.
