What you'll do

• Evaluate speech, language, cognitive, and swallowing function
• develop and implement individualized plans of care
• provide evidence-based therapeutic interventions
• assess home environments and recommend adaptive equipment or communication aids
• educate patients and caregivers
• monitor progress and adjust treatment
• communicate changes in condition to physicians and the care team
• document care in the EMR
• collaborate with interdisciplinary teams and participate in case conferences, discharge planning, and care coordination.
