What you'll do

• Coach and support RN Care Managers
• review resident care practices, assessments, service plans, and medication management
• monitor clinical quality and implement improvement plans
• support admissions, transitions, condition changes, and end-of-life planning
• conduct audits and mock surveys
• assist with state surveys, investigations, and corrective plans
• train and develop clinical leaders
• collaborate on clinical systems and workflows
• investigate incidents and support resident safety
• participate in an assigned on-call rotation and travel regularly between communities.
