What you'll do

• Engage people experiencing homelessness through street outreach and walk-in services
• develop care plans and provide follow-up and advocacy
• connect clients with housing, food, healthcare, and social services
• support crisis intervention and de-escalation in complex cases
• guide less experienced staff
• develop client resources and partnerships
• collaborate with Enhanced Care Management, Behavioral Health, Substance Use, and Referrals programs
• assist with housing applications and healthcare access
• maintain EMR and HMIS records
• conduct wellness checks and case updates
• train new staff.
