What you'll do

• Provide care for patients with acute and chronic illnesses
• take full accountability for patient care and outcomes
• engage with hospitalists for hospitalized patients
• perform assessment, diagnosis, treatment, management, education, health promotion, and care coordination
• lead care teams
• engage with transitional care teams for homebound patients
• actively participate in center management and recruiting
• supervise and collaborate with Advanced Practice Practitioners
• perform other duties as assigned.

What we're looking for

Education

MD or DO

Experience

MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required. Board certification preferred, Board Eligibility required. Must have a current DEA number. BLS certification required within 90 days.
