What you'll do

• Conduct and complete MDS assessments
• evaluate resident condition and medical data
• prepare and electronically transmit reports to Medicare/Medicaid
• develop written plans of care
• provide information to residents/families on financial assistance
• ensure MDS notes are informative
• assist with discharge planning
• implement committee recommendations.
