What you'll do

• Lead and support clinic teams in day-to-day priorities
• oversee execution of the Hopscotch care model
• manage provider administrative tasks, panels, and documentation
• partner with the Care Center Manager on performance metrics and outcomes
• provide high-quality care to a dedicated patient panel
• oversee the clinical care team
• support talent, culture, clinical innovation, and compliance
• represent the organization in community education and events.

Why this role
