What you'll do

• Follow each resident’s individualized service plan
• observe, report, and document changes in condition and daily care
• record assigned vital signs and weights
• assist with activities of daily living and restorative techniques
• provide behavioral prompts and support
• help residents attend activities, transfer, ambulate, and travel within the community or to outside appointments
• respond to urgent resident calls
• support other departments
• maintain clean and safe areas
• change linens and provide laundry
• assist with resident arrivals and transfers
• attend training and staff meetings.
