Care Manager, Community Adult Program

Job overview

The Care Manager, Community Adult Program role is full-time and community-based in the Denver Metro area, with extensive travel throughout the service area. Compensation is $26.44–$31.25 per hour. Paragon Behavioral Health Connections provides person-centered behavioral health services across Colorado. The care manager provides intensive coordination and practical support to adults with serious mental illness who face barriers to traditional outpatient care. The role helps clients access housing, healthcare, benefits, and community resources, build recovery skills, and transition toward less intensive services.

What you'll do

• Provide community-based care management and outreach
• coordinate behavioral health, medical, housing, employment, and social services
• help clients identify goals and obtain documents, benefits, and resources
• support coping skills, wellness, medication adherence, and appointments
• assist with housing stability and applications
• coordinate healthcare and public benefits
• complete required documentation
• support transition and discharge planning.
