Director of Community Health

Job overview

The Director of Community Health role is onsite in Greensburg, Indiana. Pay is $55,000–$65,000 per year. The position is fully funded by the Indiana Department of Health Rural Health Transformation Grant and supports the Decatur Shelby County YMCA’s community health initiatives. The director leads the development, delivery, evaluation, and growth of the Prescribe the Y program and related chronic disease initiatives. The role coordinates staff, healthcare partnerships, referral pathways, data, and grant deliverables to support participant health and program sustainability.

What you'll do

• Lead design, implementation, and improvement of Prescribe the Y and related initiatives
• develop standardized protocols, assessments, intervention plans, and workflows
• recruit, supervise, train, and evaluate program staff
• build healthcare and community partnerships and referral pathways
• oversee participant data, outcomes reporting, privacy, and program evaluation
• monitor engagement, retention, completion, and health outcomes
• coordinate delivery with wellness and branch leaders
• manage grant deliverables, reporting, and budgets
• represent the YMCA in community health initiatives.
