What you'll do

• Receive resident referrals and follow the admissions process
• complete bedhold calls
• manage Medicare postcard mailings and recipient records
• conduct facility tours
• screen prospective patients for level of care, services, equipment, and insurance coverage
• coordinate and authorize admissions
• obtain level-of-care approvals, screenings, and payer authorizations when applicable
• coordinate pre-admission services with clinical staff
• welcome residents and families
• obtain referral documentation
• coordinate room assignments with Nursing
• deliver census and month-end reports
• track referral activity in the PCC CRM.
