What you'll do
- Provide clinical oversight and case management as required
- coordinate client care and transitions
- facilitate caregiver orientation and competency validation
- ensure caregiver skills match client requirements
- supervise caregivers and field staff
- review documentation
- educate team members, patients, families, caregivers, field staff, and referral sources
- participate in clinical quality initiatives
- conduct comprehensive client assessments
- develop and update individualized plans of care
- provide interventions
- administer medications as needed
- maintain client records
- and initiate referrals as needed.
