What you'll do
- Meet with clients weekly for care coordination and aftercare planning
- assess needs, strengths, and barriers
- develop care plans with clients and the treatment team
- facilitate psycho-educational groups
- identify whole-health and social-determinant needs
- conduct post-discharge follow-up
- coordinate referrals and services
- transport clients to appointments and meetings as needed
- complete documentation
- coordinate discharge planning
- attend staff meetings and trainings
- assist with crisis management.
