The role contributes to recovery and wellness through evidence-based care, coordinated treatment, crisis support, and progress monitoring.

What you'll do

• Conduct integrated assessments and ongoing evaluations
• develop person-centered treatment plans
• provide individual and group counseling using evidence-based practices
• coordinate care with internal and external providers
• provide crisis intervention
• maintain clinical documentation
• monitor progress and adjust treatment
• support coping skills, relapse prevention, and long-term recovery goals.

Why this role
