What you'll do

• Provide outpatient primary care to an adult, senior-focused, or mixed patient base.
• Build long-term patient relationships, focus on preventive care and chronic disease management, and support continuity of care.
• Practice with clinical autonomy in a physician-led, value-based model and collaborate with medical assistants, care coordinators, and administrative staff.
