Social Services Manager

What you'll do

• Assess psychosocial status related to illness and environment and communicate findings to the registered nurse
• conduct social evaluations and plan person-centered interventions
• maintain clinical documentation
• coordinate referrals and provide information about practical, environmental, and community services
• serve as liaison with community agencies
• collaborate with facility staff on care plans and case conferences
• support behavior monitoring and interventions
• participate in discharge planning
• coordinate medication reviews to support gradual dose reduction where appropriate.
