What you'll do

• Provide home health nurse visits
• conduct comprehensive assessments and document them in the agency portal
• administer at-home IVs and provide wound care
• obtain and document physician orders
• initiate, evaluate, and revise plans of care with the Clinical Team Manager
• coordinate with physicians and home care team members
• complete recertifications, resumptions of care, transfers, and discharges as requested
• assess need for other team services
• conduct Home Health Aide supervisory visits.
