What you'll do

• Assess counseling needs and substance use diagnoses using DSM criteria
• determine appropriate level of care using ASAM criteria
• obtain relevant records
• prepare and provide counseling services in treatment plans
• develop discharge and continuity-of-care plans
• document assessments, services, referrals, and follow-up
• maintain a patient caseload
• coordinate support services
• facilitate family therapy as assigned
• participate in clinical meetings and crisis interventions.
