What you'll do

• Conduct skilled nursing visits, including admissions, revisits, and discharges
• provide direct patient care, assessments, and education in clients’ homes
• collaborate with the interdisciplinary team. Requires a current unencumbered Rhode Island RN license, CPR certification, reliable transportation, a current clean driver’s license, and automobile insurance.
