Care Coordinator — Oncology

Job overview

The Care Coordinator role is onsite in Providence, RI, five days a week, with a schedule of 8:30 a.m. to 5:00 p.m. Compensation is $50,000 to $60,000 per year. Daymark Health provides personalized cancer care navigation, symptom-focused support, behavioral health care, and social resources through a hybrid in-person and virtual care model. The coordinator supports whole-person oncology care by connecting patients with social and community resources, arranging follow-up care, and coordinating with external providers. Timely documentation and follow-up help patients navigate care and enable the clinical team to deliver coordinated support.

What you'll do

• Conduct structured social-needs assessments and connect patients with community resources
• support care plans and participate in case conferences
• prepare patients for virtual visits and provide basic technology troubleshooting
• schedule follow-up visits and facilitate referrals
• prepare charts for providers
• track symptom surveys, follow up, and escalate clinical concerns
• document interventions and coordination activities in the electronic health record.

Why this role

At least three years of healthcare navigation, patient support, clinical, or care coordination experience is requested. Occasional patient-home visits may occur; bilingual skills are a plus. Patient-facing roles require BLS certification by the start date.
