What you'll do

• Provide nutrition counseling, diabetes education, weight management strategies, and meal planning.
• Coordinate with Community Health Advocates and providers to identify and enroll patients in the Food Rx program.
• Monitor patient progress and incorporate evidence-based strategies for population health management.
