The Assessment Nurse Care Coordinator – Per Diem is an onsite, as-needed role in Boston, MA, with weekday hours between 8:30 a.m. and 5 p.m. Compensation is $49–$55 per hour based on experience. Boston Health Care for the Homeless Program provides health care to people and families experiencing homelessness in greater Boston. The registered nurse coordinates care for clients enrolled in the OneCare Program, conducts assessments, and develops individualized care plans. The role connects clients with providers and eligible services, conducts face-to-face outreach, and documents interactions and authorizations in electronic medical records.

What you'll do

• Coordinate care between BHCHP medical teams and OneCare
• conduct Uniform Core Assessments
• collaborate with primary care and interdisciplinary teams
• conduct outreach and assess client needs
• communicate with clients, caregivers, providers, and shelter partners
• document interactions and outcomes in electronic records
• enter service authorizations and bill assessments.
