Community Health Worker

Job overview

The Community Health Worker role is an onsite position in Elk Grove, CA, with a Monday–Friday schedule from 9:30 AM to 6:00 PM. Compensation is $21–$23.19 per hour. Turning Point Community Programs provides community-based support focused on crisis resolution, wellness, and access to services. The worker partners with members to support their stated recovery and wellness goals, reduce barriers to care, and connect them with health and social resources. The role uses lived experience and training to support advocacy, community participation, and access to preventive and ongoing services.

What you'll do

• Support recovery principles and tools
• model personal responsibility, self-advocacy, and hopefulness
• work with members to identify strengths, challenges, and goals
• support recovery planning
• connect members with health and social supports and community resources
• support wellness activities
• schedule in-person meetings with care coordinators
• provide immediate support consistent with program philosophy
• support health and safety practices
• arrange transportation and accompany members to office visits when necessary
• advocate with healthcare facilities
• support medication adherence and treatment plans
• distribute health promotion materials and complete paperwork.
