Primary Care Physician

Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient. Takes full accountability for patient care and outcomes. Engages with hospitalists when patients are hospitalized. Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
