Social Worker

The Social Worker role is onsite in Fresno, CA, and pays $25–$30 per hour. The part-time schedule is Monday through Friday, 8:00 a.m.–2:00 p.m. Fresno CBAS serves adults with physical, cognitive, and/or developmental disabilities throughout Fresno County and describes its work as providing culturally sensitive care and support. The social worker coordinates participant services, completes assessments and person-centered care plans, supports discharge planning, and documents progress. The role also provides counseling and group support as needed, supervises social work support staff, and connects participants with community resources.

What you'll do

• Manage screening and intake and follow up on inquiries
• meet with families or referring agencies to determine appropriate programs
• conduct in-home assessments and complete assessments, reassessments, and quarterly reviews
• implement person-centered care planning
• provide discharge planning and care coordination
• document interventions and progress
• supervise a social worker assistant or aide
• provide group sessions and counseling as needed
• support participant rights and grievance procedures
• facilitate caregiver support groups as assigned
• provide in-service training when needed
• collaborate with community organizations on referrals.

Why this role

A bachelor’s or master’s degree in Social Work from an accredited program is required. Active California licensure (ASW, LCSW, or eligibility to obtain) is preferred. Bilingual English/Spanish is a plus; current first aid and CPR, a California driver’s license, reliable private vehicle, and current vehicle insurance are listed as employment conditions.
