What you'll do

• Provide medical evaluations, diagnoses, and treatment
• conduct initial, routine, and follow-up assessments
• develop and revise individualized treatment plans
• manage acute and chronic conditions
• order and interpret diagnostic tests and laboratory results
• prescribe and monitor medications
• evaluate urgent medical concerns within scope of practice
• participate in care planning and family consultations
• coordinate with care teams and community resources
• educate patients, families, and staff
• document care in the EMR
• contribute to quality improvement.
