Primary Care Physician — Dallas–Fort Worth

The Primary Care Physician role is onsite, providing home-based care across the Dallas–Fort Worth, TX territory. Compensation is $225,000–$260,000 annually, depending on experience, with potential performance-based bonuses. Bloom Healthcare provides care to medically complex patients where they live. The physician manages a defined patient panel through longitudinal, relationship-based primary care, acute care and specialty assessments. The role contributes to improved health outcomes, continuity of care and reduced readmissions through coordination with interdisciplinary teams and timely post-discharge visits.

What you'll do

• Provide longitudinal primary care in patient homes and facilities
• diagnose, manage and treat acute and chronic conditions
• address social determinants of health
• conduct wound care, post-hospital transitional care and condition-specific assessments
• complete evaluations including medication reconciliation, fall-risk assessments, advance care planning and health education
• use integrated data to track hospitalizations and coordinate post-discharge visits within 72 hours
• collaborate with care teams to support continuity, reduce readmissions and stabilize high-risk patients.
