What you'll do

• Manage relationships and performance outcomes for assigned primary care practices
• lead data-informed performance conversations and improvement plans
• influence adoption of workflows
• identify risks and intervene
• translate performance data into recommendations
• present findings to stakeholders
• guide practices through HEDIS, MIPS, ACO, payer, and incentive initiatives
• coordinate with internal teams, payers, and practice stakeholders
• prepare executive reporting.
