Home Health Community Liaison / Marketer

The Home Health Community Liaison / Marketer role is based in Sterling Heights, MI, with regular visits to referral partners and communities across Southeast Michigan. Compensation is $32,500–$65,000 per year. Classic Home Care is a nurse-owned, Medicare-certified skilled home health agency serving Southeast Michigan. The liaison builds referral relationships, educates partners and the community about home health services, and supports patient transitions into and back to home care. The role also tracks outreach and referral activity and identifies partnership opportunities to support service growth.

What you'll do

• Build and maintain relationships with hospitals, physician offices, skilled nursing facilities, assisted living communities, and other referral sources
• educate partners and the community about services
• visit current and prospective referral partners
• conduct welcome and continuity visits
• represent the agency at community events and health fairs
• collaborate with clinical and administrative staff on patient transitions
• track outreach, referral data, and market trends
• identify partnership opportunities
• manage social media.
