Direct Support Professional

The part-time Direct Support Professional role provides approximately 20 hours per week of onsite, in-home and community support in Schuylkill Haven, PA. What you'll do

• Provide one-on-one support guided by the participant’s Individualized Support Plan
• transport the participant to activities, appointments, and outings
• assist with personal care, meals, and community participation
• promote independence and social inclusion
• monitor and report changes in health or behavior
• document progress
• maintain confidentiality
• collaborate with the care team and family
• and complete required Pennsylvania ODP training and certifications.
