MDS Care Plan Coordinator

What you'll do

• Complete accurate resident assessments and interdisciplinary care plans on time
• identify resident acuity and needs and communicate care expectations to families and responsible parties
• integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services
• complete Medicare/Medicaid case-mix documents
• conduct ongoing MDS reviews
• oversee tracking and submission of MDS/PPS documentation.
