What you'll do

• Maintain contact with acute care referral partners, including discharge planners, case managers, physicians, hospitals, and nursing staff
• receive, track, and follow through on hospice referrals
• evaluate patients and coordinate or complete admissions as assigned
• explain hospice services and care options to patients and families
• complete admission documentation
• coordinate with hospital case managers and hospice teams to support timely admission and safe transitions.
