Social Services Specialist

The Social Services Specialist is an onsite, full-time day role in Denver, CO; compensation is $20.04–$24.23 per hour. The position supports residents and families at Wheatridge Manor Care Center, with direction from the Director of Social Services. The specialist helps residents and families adjust to placement, illness, and care plans while supporting residents’ rights, psychosocial needs, and access to services. The role contributes to person-centered care through assessment support, resource coordination, documentation, and discharge planning.

What you'll do

• Assist with resident and family interviews and psychosocial histories, assessments, and care plans
• support transitions and adjustment to placement
• provide information about rights and advance directives
• help arrange evaluations and appointments
• connect residents and families with community resources
• participate in councils as requested
• support reimbursement applications and theft prevention
• document care
• assist with discharge planning and follow-up services.

Why this role
