Through outreach, documentation, and coordination with patients, families, caregivers, and providers, the associate helps connect people with appropriate primary care services.

What you'll do

• Conduct telephonic and in-person outreach to prospective, high-priority patients
• guide them to Hopscotch services or help them find alternative primary care options
• spend approximately 10% of time engaging patients in community settings, including homes, hospitals, long-term care facilities, and skilled nursing facilities
• document interactions promptly while maintaining HIPAA standards and confidentiality
• coordinate with patients, families, caregivers, and healthcare providers.
