What you'll do

• Provide in-person hospice care during after-hours and weekend shifts, including assessments, admissions, routine visits, death visits, and live discharges
• assess physical, emotional, psychosocial, and spiritual needs
• coordinate with the Interdisciplinary Group, physicians, families, and hospice staff
• develop and revise care plans
• educate patients and families on disease progression, symptom management, medications, and hospice philosophy
• document care
• support workflow audits, survey readiness, quality activities, and regulatory compliance
• guide field staff when appropriate.
