What you'll do

• Develop and maintain relationships with local hospitals, healthcare providers, and stakeholders

• participate in discharge planning and care conferences
• collaborate with interdisciplinary teams
• educate and support patients, families, and caregivers on discharge plans and post-acute care options
• maintain accurate records and reports in accordance with requirements and facility policies.
