What you'll do

• Provide skilled nursing services in patients’ homes under established plans of care
• assess and document physical, psychosocial, and environmental needs
• reassess conditions and report findings
• implement physician orders and nursing interventions
• provide pain management and medication education
• follow up on laboratory results
• collaborate with the interdisciplinary team
• complete clinical documentation
• participate in discharge planning and case conferences
• coordinate referrals and supplies for home care.
