Housing Case Manager I is a full-time, onsite role at 2550 Main Street in Buffalo, NY. Pay is $19.00–$22.26 per hour, with a $2,500 sign-on bonus. Spectrum Health & Human Services partners with adults, children, and families affected by behavioral, emotional, mental health, or substance-related disorders. The case manager supports adults who are homeless or at risk of homelessness in obtaining and maintaining safe, affordable housing and residential stability. The role coordinates housing searches, benefits, service providers, families, landlords, and community supports while maintaining required program records.

What you'll do

• Help individuals locate safe and affordable housing in an area of their choice
• support progress through program phases and housing goals
• assist with household furnishings and benefits
• collect income and diagnostic information and complete rental stipends
• communicate with treatment and service providers, families, landlords, and community supports
• conduct outreach
• maintain progress notes and required program records
• meet with individuals in person twice monthly or more often as needed
• attend team and community meetings.
