What you'll do

• Oversee Social Services operations in accordance with NASW ethics and applicable regulations
• complete psychosocial histories, assessments, and relevant MDS sections
• participate in interdisciplinary care planning
• support resident rights, mental health, and adjustment
• maintain compliant documentation
• educate and support residents and families through placement, adjustment, discharge planning, and community-resource access
• coordinate referrals and post-care arrangements
• assist with policy development and survey readiness.

Why this role

Missouri social worker certification, license, or registration and a bachelor’s degree in Social Work or Human Services are required. Requires at least one year of supervisory social work experience in healthcare with geriatric populations; two or more years of skilled nursing facility experience preferred.
