Licensed Vocational Nurse Clinical Intake Coordinator

Job overview

The Licensed Vocational Nurse Clinical Intake Coordinator role is onsite in Elk Grove, CA, with a Monday–Friday schedule of 40 hours per week; varied hours, including evenings, nights, weekends, and overtime, may be required. Pay is $35–$40 per hour. The role supports Home Health operations within a patient-centered healthcare organization. The coordinator manages referrals, authorizations, scheduling, patient records, and communications to help patients receive timely Home Health services. The role connects patients, families, providers, referral sources, and Home Health staff while supporting accurate documentation and compliant intake operations.

What you'll do

• Coordinate Home Health services and orders
• receive referrals and manage intake with involved parties
• coordinate IV therapy with vendors as needed
• schedule patient visits and services
• enter patient information into electronic medical records
• audit admission paperwork and maintain records
• make welcome calls
• assist with authorizations and explain coverage and financial responsibilities
• respond to calls and inquiries
• provide administrative support to the Home Health Manager
• protect patient privacy and follow documentation and regulatory requirements.

Why this role

Current, unencumbered Licensed Vocational Nurse license and graduation from an accredited nursing school are required. At least two years of patient intake or medical office administration experience is required; bilingual English/Spanish is preferred.
