HIV Clinical Care Coordinator

Job overview

The HIV Clinical Care Coordinator role is onsite at the PHC University Campus in Des Moines, IA, typically Monday through Friday, 8 AM–5 PM, for 40 hours per week. The stated hiring range is $18.65–$23.32 per hour; candidates with extensive related experience may be considered up to the pay range maximum. Primary Health Care supports patients living with HIV through a Ryan White funded grant program. The role coordinates care for high-acuity patients, helps resolve barriers to receiving care, and provides administrative and delegated clinical support in collaboration with the multidisciplinary care team.

What you'll do

• Coordinate care and referrals for patients living with HIV
• identify and address barriers to care
• respond to patient inquiries in clinic and through communication channels
• complete prior authorizations
• facilitate rooming and intake, obtain vital signs, assist providers, and perform point-of-care testing
• administer medications, immunizations, and treatments under orders
• document patient data and grant service units
• complete quality controls and follow infection-control procedures.
