• Assess patients and families using examinations, medical histories, and OASIS data
• develop and evaluate care plans
• provide skilled nursing during home visits
• administer prescribed treatments
• educate patients and families
• monitor and report changes
• document care in electronic health records
• identify discharge planning needs
• participate in meetings and performance improvement
• follow infection control and HIPAA requirements.
