• Coordinate and oversee patients’ home care

• communicate patient progress to the clinical manager, physician, and care team.

Why this role

Current state Registered Nurse license, driver’s license, and CPR certification required. Two years of clinical RN experience and one year of full-time hospice experience are preferred.
